Payment Responsibility
Medical insurance payment problems are between the patient and their insurance company. 

After payment or denial from your insurance company, any outstanding balance will be the patient's responsibility.
Co-pays are payable at the time of the visit.

Our Billing/Collection Department can arrange payment plans if you require.  Please make arrangements with the Billing/Collections Department before your visit.
